Knox County Community Health Center Earns NCQA Patient-Centered Medical Home Recognition

Knox County Community Health Center has achieved National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition, a distinction that reflects the health center’s commitment to providing coordinated, accessible, and patient-centered care.

The recognition highlights key areas of care, including team-based care, patient-centered access and continuity, care management and support, care coordination and transitions, knowing and managing patients, and performance measurement and quality improvement.

The PCMH model places patients at the center of their care by strengthening relationships between patients and their clinical care teams. Research shows that PCMHs can improve quality, patient experience, and staff satisfaction, while helping reduce health care costs. Practices that earn recognition demonstrate a commitment to quality improvement and a patient-centered approach to care.
